Subject: Flu Vaccination Catch-Up Session – Important Information

Dear Parent/Guardian,

Our Immunisation Team will be returning to your child’s school on 14th November 2025 to offer another opportunity to receive the flu vaccination.

 If you haven’t already, please complete the online form to record your wishes:

• To decline the vaccine, please still complete the online form. This helps us update our records and avoid sending further reminders.

 You only need to submit one form. Your consent (or refusal) will stay in place for the whole flu season.

If your child has already received the flu vaccine during our previous visit in September 2025, or at another vaccination clinic or school, please do not complete another consent form (either paper or electronic).

During the 2 week period that our school is adopted, a variety
of perks will be available to parents/guardians, school pupils
& school employees, these will include the following:

• FREE Junior Gym & Swimming for Pupils (Aged 11-15)
• FREE Membership for Parent/Guardians (Aged 16+)
• FREE Membership for School Employees (Aged 16+)
